I have spent my entire professional career serving older adults in multiple capacities. Social work in long term care and admissions work in skilled nursing inspired me to create Highest Level Care Coordination. My passion is to help older adults and their caregivers thrive to the best of their ability. It is tough to navigate the health care system, particularly for older adults with medical complexities, any type of memory changes and or little to no support systems. It has proven to be highly beneficial for the individuals and families I serve. I am their professional available to help navigate the healthcare system and other barriers. I can help anyone, as an addition or in place of natural support systems.

Care Coordination for older adults and their families can be a myriad of different services from: advocacy with medical care needs, communication with medical teams, setting up safe discharges home and resources appropriate to individuals. Many of the clients I've served, describe me as a professional family member. Each person's services are individualized to their specific needs.
